you can go to google and search "medicann" and it shud be the first thing that pops up. You call the number and request an appointment. It's 150 bucks for the appointment (whether or not ur approved). You will need recent records of your diabetes diagnosis with u, and your sate ID. They will tell u where the clinic is and everything.
