Citizens within Islamic states are subjected too harsh punishment even death penalties if they criticize or leave ideology of Islam. almost as Christians were in the MIDDLE AGES.

Yes they very much need to update , there is no doubt about that , because Islamic states have not adopted "Secularism" (the separation between government and religious institutions) their government is ruled by Sharia Law , which is harsh islamic law that saturates every aspect of the government.
