I've seen videos of surgeons using something called "Alloderm" instead of cutting into the patient's palate. Why don't all surgeons use this? and what are the advantages and disadvantages of both techniques? I'm asking this because using Alloderm seems to be a lot trouble to use and less discomfort for the patient. Thank you.
The reasons many surgeons still use connective tissue grafts are several.
1 First, many patients refuse to have tissue from a cadaver placed in their mouth.
2 Just like autogenous bone is the "gold standard" for bone grafting because of osteogenesis from the patient's own cells, the same applies to soft tissue. The autologous soft tissue graft has the best regenerative potential and the highest success.
Another reason, although not critical, the allografts like alloderm or Puros Dermis (which is better than alloderm and does not require refrigeration) is that they increase the cost of the procedure for the patient.
In right excursion the molars will bump into each other. The patients canines are shorter than necessary. Further perio problems are expected. Dr Sanjay Jamdade
I don't know how you can diagnose an occlusal premature contact in lateral excursions from a lateral intraoral photographic view. I assure you that there is no such problem. Not that it would matter, since the recession is due to a combination of very thin tissue biotype and a very thin labial plate on the canine due to orthodontic expansion.
