If Cisco used the Silvertel Ag6400 (4-channel PSE controller module) reliability would be higher, and they could also get up to 30W per RJ45 (IEEE802.3at).
I hate this video. I'm the guy that has to repair these units. And seeing the end user making the repair harder to fix makes me want to cry lol. Get one PoE controller down and it shuts all PoE down.

However, we are constantly RMA'ing 3550's and 3750's with various issues so taking one out of the lot to play with for 5 minutes isn't too bad :P The 3550 PSU issue was driving us nuts there for a few years. We were having to replace one a week!!!
hmm well cisco can kinda suck When I worked at verizon wireless we use a cisco router I will not mention names but it cause a few of our Kiosks to burn down sometimes part of the malls they were in..
Hmm, cisco did a HUGE recall on that, it was a design error. The POE caps were too close to a transistor heatsink, and the original japanese capacitor vendor's caps worked fine, it was until they introduced a lower cost capacitor which had issues dealing with heat.
lol or you can do what i do and just RMA the damn thing to cisco and assign it a priority 1 to get a replacement the next day.
we mainly have issues with 4506 blades going bad.
the ports begin to flap slowly until the 8 go bad. then the next 8. until the entire blade is completely down. happens quiet frequently. once or twice a year. for 1 blade.
but only on some 4506. it seems to be the actually chassis.
thats funny "Those damn tantalums again" I had that very same thought. Just the slightest over voltage and they sizzle. They don't seem to react very well to rf fields either.

Do flaming caps shoot out like little rockets exploding from their top or side?
I've seen that happen when the $9 AT PSU in a $24'000 flatbed scanner fried.
Then the fucks at the company wanted $1700 to repair that hazard.
Instead I fixed it myself and reported them for making a hazardous product.

I found they would short out sometimes in tv sets...one particular model would lose the tuner when this happened.
