The pilot made a blunder. The effect of strong ebb current and shallow water was underestimated. On the current in narrow fairway the vessel cant turn with the same turn rate as at sea. Most of the pilots are competent and responsible navigators. However this case proves one more time that the Master and OOW should control the activity of the pilots very carefully.
