Lol, this is definitely not playback. If it were playback, they would have used the studio track of the vocals, and Gerard's vocals here are different from the studio version. It's only an amazing live performance.

Also, when he takes his mouth away from the mic and the vocals are still in range it's because he uses mics with volume equalizers on them. They get a bit louder when vocals are taken out of range, and quiter but clearer when the mouth is closer.
