If you have symbian s60v2 or s60v3 try use UnlockMe! 1.0.

It helps in my case. Just download this program then instal on your mobile phone and run. Program try to gues your code. It takes for about 2 minutes. When program will find the code you will hear "beep". Write it down and use it to make change or remember.

Remove the sim, restart the phone and select offline mode. Once the phone boots up, power off the phone. Replace the sim back into the phone and restart the phone. Once the phone loads change the active profile to General and your phone service will switch back on.
