Rogues had a Talent called "Unfair Advantage" that caused you to attack with your main hand for 100% dmg when you dodge, You could also with the right gear Cap your dodge at 95%. Aye. It was a Level 60 raid that came out in Patch 1.11. It was changed in 3.0.2 for Level 80.

Now, I won't spam this comment but let me tell you, there was Tier 3, the best tier of all time which was hard to obtain back in the old days. The casuals who raided Naxxramas used to get that kind of tier.

Even the loading screen tips specifically say "Npcs can dodge from behind but not players" and it's true that you can not do so as I have personally played on a private server and gotten a 1k defense ring. The second verifying fact is that you did not get a debuff from stepping into the green slime witch is not avoidable, because it's not scripted.
