In my opinion i think in the end of cod 4, gaz was clipped on the head with the bullet and survived. But it could have affected his facial ability , when he seen how it affected him he started to become embarrassed and made his mask. After that he could have made contact with Soap saying he was coming back in service but dont call him Gaz as there could have been rumours about the whole Zakhaev deal which could have made his name public so Soap nicknamed him ghost .
