When you get to star road, unlock the star in the middle, that takes you to the special stages, beat all the stages and teleport out, lots of stuff gets changed

Absolutely. Its the only way to get the maximum score of 96 at the save select screen.

Those numbers represent a progress of sorts and until you complete the special stages and unlock the "other" world with swapped around enemies, you'll never fully complete the game 100%.

There is no extra anything associated with it, besides changed color schemes in the various worlds and changed around enemies. So it's all just bragging rights as far as that is concerned.

back door is just a much shorter version of bowsers castle. It only has the last room as well as a midway point by bowsers door. When you complete the last level of special world then 3 enimies change and they are the koopas, bul;let bills and the jumping parana plants. It also changes the colours of the overworld maps.
