You don't understand the basics of diagnostic testing. Elisa is very sensitive, western blot very specific. If you get a positive Elisa, its followed up with either another Elisa or a Western blot. The combined accuracy is very high. If anyone doubts what I say, please check the facts with an HIV physician or expert advice service. (or on wikipedia for that matter).

If a test has a 0.1% chance of creating either a false positive (someone is really HIV negative) or a false negative (someone who really is positive) that 1 person out of 1000 (0.1% of 1000) doesn't really change the validity of the test, it is still giving accurate results to 999 people out of the 1000..... did I miss something?

Although RNA PCRs are done, they are useless, because the virus enters as RNA (it is only active when it has integrated into the host's genome). THUS we look at HIV DNA .. DNA PCR: which quantifies the number of times the DNA was exponentially multiplied.
But in PCR, from the RNA cDNA is synthesizes. So you aren't looking at RNA anymore.

Med school isn't really all that difficult if you work reasonably hard. If you want to give yourself a leg up, go ahead and start learning micro. Get a hold of a good micro for dummies text & learn what is gram +, gram -, mycobacteria, etc. Then go ahead and learn anatomy. Bones, muscles, ennervation. Also get a neuro coloring book. Sounds stupid, really helps. If you start now, by the time you get into med school, those will be easy.
And start taking practice MCATS sooner, not later.

You don't understand the basics of diagnostic testing. Elisa is very sensitive, western blot very specific. If you get a positive Elisa, its followed up with either another Elisa or a Western blot. The combined accuracy is very high. If anyone doubts what I say, please check the facts with an HIV physician or expert advice service. (or on wikipedia for that matter).

I'm a bit confused, David states that with a test that is 99.9% effective there would be 50% false positive test results and thus the test is meaningless. If a test has a 0.1% chance of creating either a false positive (someone is really HIV negative) or a false negative (someone who really is positive) that 1 person out of 1000 (0.1% of 1000) doesn't really change the validity of the test, it is still giving accurate results to 999 people out of the 1000..... did I miss something?

ACtually my buddy, PCR for HIV is used to quantify DNA. Although RNA PCRs are done, they are useless, because the virus enters as RNA (it is only active when it has integrated into the host's genome). THUS we look at HIV DNA .. DNA PCR: which quantifies the number of times the DNA was exponentially multiplied.

My understanding of the shift in tropisms is that the cxcr4 tropic viruses are more effective at killing cells, and the ccr5 tropics are more effective at replicating.
And it is my understanding that r5s predominate in 1ary infxn.
But nice to actually talk to someone who has a clue.

Not me. But my buddy does some research on the side, and we talk about it all the time. We are ER docs in Atlanta & his research topic before getting into med school was HIV tropisms. His screen name is CCR5CXCR4, ironically.
MABS to block CCR5 or using adenoviruses to deliver zinc fingers to alter the genes for ccr5 are showing some promise.
I do not know why MABs to cxcr4 has not been effective, but that has not shown the same promise.

Really, I did not know that this was being done.
But I wouldn't be surprised that X4 wasn't as promising. We always use HIV-Bal cuz it is R5 and shows more consistent results.

Med school isn't really all that difficult if you work reasonably hard. If you want to give yourself a leg up, go ahead and start learning micro. Get a hold of a good micro for dummies text & learn what is gram +, gram -, mycobacteria, etc. Then go ahead and learn anatomy. Bones, muscles, ennervation. Also get a neuro coloring book. Sounds stupid, really helps. If you start now, by the time you get into med school, those will be easy.
And start taking practice MCATS sooner, not later.

I hated organic. But I kicked ass at physics and the rest. Biochem can be a beast. You need to do well in your u-grad courses, but for m-school, the key info are the rate-limiting steps and cofactors required.
Ex: Pyruvate dehydrogenase requires what cofactors, and why do we care? Well we care, because PDH is a key enzyme linking glycolysis and the citrate cycle (krebs). If you are lacking in a cofactor then your oxidative respiration will be compromised (can be fatal).

Answer is: Not every single virus mutates in the same exact fashion. Lets say a virus mutates and its Tat gene is not functional. That means that particular viral particle dies. However, the rest of the virues don't.
Reason why they have remained unchanged is because they are TOO vital. Although I am sure that polymerase I can't detect what is important, thru natural selection, the virus that are way too mutated are eliminated, thus those that function are the ones that are left! Explained

there are true tests that prove you have HIV... simple t cell count. Viral load. p24 amounts. PCR on its genes (tat, rev, nef and so on) TLR 3, 4 7 8 9 stimulation.. and much more..
but yes.. since HIV does thrive in inflammation, the drugs that usually are made for it end up hurting ppl..
HIV mutates a lot, so its hard to find anything that works, unless you help it mutate so much that it becomes inactive.

Heterossexuals and Homossexuals both have HIV infection, there is no point in distinguish between the 2. both might get HIV- AIDS

Yeah...you confirmed that "hiv" rates have remained VERY low in a country where there are tons of hookers and people can't be persuaded to buy condoms.

do you even know that HIV has increased in older couples due to the fact that the man goes to prostitues?

gp120 and gp41, are not nonspecific, they are 99% indicators of HIV infection, i gave you the references already
