this is how it is done, She is in on it, she has lost a tooth their anyways and a prosthetic is put in. As david is fondling her teeth he puts a sort of spring tooth thing (like the "Biting the coin then blow it back" trick) then as he blows the same thing happens. its quite clever really.

The girl is wearing a set of dentures or something custom made for the trick with removable teeth under her real teeth, She is just using her upper lip to keep her real teeth covered. When Dave supposedly spits the teeth back she just bites down showing her real teeth.

She was in on it. There are fake teeth that are molded to fit over her real ones. When he pulls on "her teeth" he's really unsticking them from her upper mandible (they sit there in a similar way to invisible braces). When he blows on her face, she uses her tongue to click them back into place. When she gets pushed forward by the guy to her right (who is also in on it, a trustworthy stranger to show enthusiasm) she pushes the cap out of her mouth into the guy's hand who pulls the magician back.
