If he had cancer, he'd be making jokes about that, too. It's what comedians do. And Dave is not "hollywood"--he lives in Connecticut, and his show is in NYC. He's not exactly a hollywood guy in his lifestyle or associations either. I mean, it's not like he's having sex with the guests. You're talking about a guy who drives himself to work every day.
The big problem for Letterman is that legally he's in jeopardy. The station would be within their rights to fire him for having sex with his staff. They can argue that a repeated pattern of sex between a boss and his employees creates a atmosphere of "expectation" in the workplace and is therefore coercion, even if the sex was consensual. But at least he's taking it like a man.
This has to represent some kind of cultural low for America--the guy who built an entire career and made tens of millions by being inappropriate and disrespectful has the nerve to go on national t.v. and use his lurid extramarital affair as a means to manipulate audience approval.
