It's just kind of random is you don't use cheat codes and you don't have open for business.

The cheat code to have twins is

boolProp testingCheatsEnabled true and then you shift+click on the sim you want to have twins and there will be an option call spawn and then there will be another one called force twin and click on that and you'll have twins. In Open for Business you just have them eat cheescake.
