That's actually a software, not the game itself. For sharps and flats hold shift before clicking.

The SNES can't do sharps and flats. This is Mario Paint Composer, a downloadable freeware program that is like the original, but with new features. (i.e. sharps and flats, more notes can be put on the same beat and such)
