Start with and A played with your first finger in Bar form. The High E string gets muted in the process and doesn't need to ring. To give it a little extra oomph, you can actually hammer on this form at the beginning of strumming from open strings, but again the thumb is muting the bottom E string.

If you look at what Dominic is doing with this thumb, wrapping you'll see how he's muting the bottom E a lot. From this A your 2nd and third fingers can hammer on and off to make a D.
