  • @GlobalishlyGoonish I'm not an engineer, but if I understand correctly you are saying that the maintenance guys may have been told by engineers to use a forklift to drop the engine. This could be true, but the main point I was trying to make was that the pilots did not receive critical information that may have allowed them to save the aircraft. Somewhere along the line at MD or AA bad decisions were made. It may have been in the engineering or maintenance, but these guys didn't have a chance

  • Improper engine replacement procedure performed by maintenance was what caused pylon to fail, sad day in aviation.

  • All commercial pilots are well trained to deal with an engine failure at takeoff, and can handle it without difficulty. Throw in the retracted slats on one side and having the stick shaker and slat disagreement alarm disabled, and every pilot who recreated this accident in the simulator could only save the aircraft if they about the problems with the slats and hydraulics. The pilots on AA 191 thought they only had a failed engine, and did not know it had fallen off and didn't have a chance. :(

  • So basically, poor maintenance and some poor design choices conspired to set the pilots up for failure. They had no way to know about the slats and their stall protection and slat warning was disabled by the lost engine. If the pilots knew exactly what had happened and received the proper warnings they had a good chance to save the aircraft. As it was, they didn't have key information and the accident was not their fault. McDonnel Douglas and American Airline Maintenance royally screwed them.

  • @Icaller1915 The maintenance workers for AA took some shortcuts when they serviced the engine that cased it to fly off the aircraft as it rotated (took off). They still should have been able to climb out and return to the airport, but there had been some design choices by McDonnell Douglas that were poor in retrospect which led to the crash. The pilots did exactly what they were trained to do and weren't at fault. In the simulator, no other pilot in the same situation was able to do any better.

  • @Icaller1915 As a result, some of the training for pilots was changed. Basically, if an engine failure occurred above minimum climb speed, pilots would would simply maintain current speed instead of the previous practice of reducing to minimum climb speed (which had been done so that in theory the renaming engines would not be overly strained).

  • @Icaller1915 It wasn't actually the engine that failed. The rear connection point where the engine pylon attached to the wing failed. The engine then swung forward and went over the top of the wing, broke off, and landed on the runway. The aircraft could fly on two engines, but the engine breaking off caused the hydraulics that held the leading slats out on that wing failed and they retracted. and the stall speed for that wing increased. The pilots had no way to know about the slats...

  • @workingstiff76 ...retracting, so when they reduced power to minimum climb, the wing with damaged hydraulics and retracted slats stalled, the plane banked hard over to one side and crashed. The slats not "locking" in place when the hydraulics failed was a bad design choice in retrospect. Also, the stick shaker and warning alarms for the slats were powered by the engine that flew off, so they did not work. Also, only that captains wheel had a stick shaker, the co-pilot did not.

  • Sad and tragic. More disturbing are ghost tales from the field where the plane crashed.

  • My guess is that the "damn!" response from the flight deck were the pilots interpreting the forward panel instruments and I'll bet they concluded, in the moment, that there WAS an engine failure (explosion, etc.) because EPR, FF, EGT, etc. would all have indicated an engine not operating (no instrument available to tell you the 12,000 engine actually severed from the wing!) and it was difficult, if not impossible, for the left seat pilot to view that engine OR the lack of an engine on that wing

  • When the engine came off, it damaged the hydraulics that drove the control surfaces (ailerons, flaps). I can't remember if the pilots were actually aware that their engine came off - the "Damn" was right before they hit the ground.

    After the crash, many pilots were put through a simulation of what happened, and even though they were told they were going to lose the engine and hydraulics in the simulation, very few of them could keep the plane going.

  • @dw75002 Partly correct. The only hyd lines that were severed were for the leading-edge devices. This caused the LE devices to retract and (because the #1 generator powered the stick-shaker) there was no stall warning. So when they slowed to V2, this stalled the left wing, while the right wing continued to lift--hence the roll to the left. FC surfaces still functioned normally

    The sim story is urban legend. A sim wouldnt be programed to sim a flap-slat failure in response to an engine out.

  • Cont. In response to this accident, pilots are now trained to fly V2+15 knotts. In that case, even if the there was a flap-slat disagreement. the plane wouldnt stall.

  • I think I just heard a DC-10 going over but it sounded like the engine failed. It sounded like a helicopter when it was really far away, then when it got closer sounded like a cross between a high-pitch modern propeller plane and an old low-pitch war plane. Anyone know what I just saw?

  • Famous crash. My friend, who is in the airforce tells me this plane was designed to be able to still take off, (and climb, and fly, a little ways ... and land) with one engine off a wing. The pilots here didn't know how to do it ... or at any rate didn't do it.

ALL aircraft certified under part 121 standards MUST be able to take off and land after "losing" an engine. unfortunately for those on board..this was a DC-10, and not a Lockheed Tri-Star. The LE slats on an L-1011 are locked together. "split LE slat" not possible. L & R slats BOTH UP or BOTH DOWN. mechanical link. On a DC-10, the leading edge slats are not mechanically interlocked Left LE slat retracted, caused roll.

  • Subsequent to the crash the NTSB, in a SIM configured just as AA191 was in at the moment the fatal roll begain, discovered that had the pilots known EXACTLY what they were facing at the moment the airplane began to invert ~ it WOULD have been possible to successfully deal with the situation.  Unfortunately, the instrument panel was lighting up like an Xmas tree, all kinds of audible warnings were going off and it was impossible, in the moment, to know exactly what they were dealing with.

  • This accident happened, really, because the DC-10 used hydrolic pressure ONLY to hold the extention of both flaps and slats. Once the engine, on the left wing, was severed ALL hydrolic pressure IN the left wing was lost. Both flaps & slats were lost and the left wing, effectively, stopped flying (lift). With the left wing in a stall configuration the aircraft (almost) immediately began its' fatal roll to the left.

  • This accident was a result of a maintenance error in which a forklift was used to raise/lower the engine on to and off of the wing. This practice was unsafe and it resulted in heavy cracking that eventually caused the engine to tear off. In the process critical hydrolics were torn apart and the aircraft was no longer controllable at all. This airplane was doomed before it ever took off; the pilots never had any chance at all.

    First, he was beyond the point on the runway at which he could stop the plane.

    Second, he likely didn't know what happened until after he was airborne -they're focused on getting off the ground...

    I also imagine some training says "gain as much altitude as you can", because in flying, altitude=time to make decisions, and to line up for another runway.

  • @filthynumber1 once you reach a certin speed, called V1 i believe, you cannot just simply touch back down. there is not enogh runway. an aborted takeoff, depending on speed and runway can be just as dangerous if not more.

  • When the lh pylong broke free of the wing, it ruptured all the leading edge device hydraulic lines. At that time there were no lockout valves installed to protect the leading edge devices. Those devices immediately retracted, causing a fantastic loss of lift on the LH wing. The RH wind leading edve devices were still extended causing excessive lift on the RH wing.. The aircraft was forced to roll left.

  • @filthynumber1 it's because the plane passed V1 which means that the pilots don't have enough runway left to stop on. All problems appearing after V1 is going to be brought up in the air.

  • @filthynumber1 He didn't have enough runway to put it back down. He was past his "go-no go speed", a speed where you have to fly, because there is not sufficient runway to land the aircraft on the remaining runway. Also the crew was not immediately aware, if at all, that the engine literally ripped off the wing, taking hydraulic lines with it causing control and wing surface problems.

  • @filthynumber1 I might be wrong but this is what comes to my mind; they decided either gain enough altitude for turn around and emergency land or gain enough altitude to cruise at idle with lower torque and land somewhere ahead (possibly an empty field or whatever). In thi case I'd fly straight ahead at idle speed and land on an empty field.

    V1 is the critical engine failure recognition speed or takeoff decision speed. It is the decision speed nominated by the pilot which satisfies all safety rules, and above which the takeoff will continue even if an engine fails.

  • @filthynumber1 because after the V1 speed you will not be able to stop on the runway and it's safer to continue with the remaining engines. Airplanes are designed to be able to do that. Sometimes, however, shit happens and you lose more than just the engine...

    2 b honest there have been way worse crashes. like the turkish dc-10 that crashed near paris or the japanese 1 with 520 victims r scarier and had more victims. ppl suffered big time b4 crashing and thats the worst that can happen 2 u. at least this crash was fast and ppl probably died in a second or 2.

  • I grew up where and when this plane crashed, oddly what is never said is that next to where the trailer park is (the crash site) are several huge jet fuel tanks, if this DC-10 hit one of those.....In fact my mother worked near the crash site and heared the crash, everyone there thought one of those fuel container's exploded.

  • 191 seems to be a very unlucky number in aviation. There have been 5 crashes with the name, "flight 191." American Airlines flight 191, Delta Air Lines flight 191, Comair flight 191, Prinair flight 191, and the X-15 test flight 191. These crashes killed 463 people all together. In other words, I'd avoid boarding a flight 191.

  • I wonder if the pilot had retracted his flaps on his right wing if that would've straightened the aircraft so that it may have crashed on its belly, with perhaps a better chance of survival, better not absolute of course.

  • Shortly after American 191 crashed, I was shipped to Germany via World Airways, and I looked out on the ramp at MaGuire AFB to see....a DC-10. I was a little nervous, but not very.

  • My Dad had a business trip scheduled that day out of ORD on AA. When the radio announcer cut in on my car to bring the news, I got a bad feeling in the pit of my stomach. Pre-cell era....I had to drive home and call my dad's office on a landline.

    He was ok. He had booked flight 191 earlier, but changed to a later flight because there wasn't enough 1st class or business class seating available for his group or something. That was the 2nd plane he missed that had no survivors.
