A Good Dentist will NOT pull a tooth until the infection is gone. and Your advice about giving a fake name/address is LAME and ILLEGAL. I have worked in Dental Clinics where more then one dentist was on staff because of the high demand for dental work and it was like grand central station

Turned out that the infection went almost all the way up to the cheek bone,
and to get rid of the infection the body had formed a several centimeters long(!) fistula (canal) in which the pus could come out!
Metronidazol tablets helped VERY much almost overnight, so it might have been some sort of obligate anaerobic bacteria which the drug works well on!
Maybe the fistula itself provided access to oxygen, which helped the process?!
No surgery was needed as it healed so fast!
