The sequel, Painkiller Overdose is a mixture of user created content and the content that was supposed to be in the PSP/PS2 versions of Painkiller. It has new weapons, and a new character (who has one liners, which some people don't like). The problem is, there's some real problems with balance in difficulty, makings some levels so difficult they just aren't fun. But there's some real great levels in there.
Painkiller Resurrection is like an experimental Painkiller game, with MASSIVE levels (there's only six of them, but they're big) with levels more similar to Serious Sam (enemies spawning at certain areas instead of in certain rooms). The problem is, it makes the game pretty buggy. But I personally loved the game. Many hated it.
Lastly, Painkiller Redemption is really badly made user created content. No substance, no imagination, no heart put into it. Don't waste your time.
So to summarize the further the series went the worse it got. I'd say stick with Painkiller Black if you want the best Painkiller has to offer. Overdose is great, but it's uneven. The other two, either you'll like or hate Resurrection, and Redemption sucks.
I love the atmosphere in Painkiller.

There's a sale on steam right now, which painkiller games are worth buying?

Painkiller Black (which is the main game and its expansion, Battle Out of Hell) is the best by far.
