The one thing that confuses me in this episode is when they originally believe that she is dead (I think they thought she was dead anyway...not sure, perhaps they thought she was in some sort of witness protection program or something?), and John goes to meet her (thinking it's Mycroft), and it turns out Sherlock had followed...does that mean he knew she was alive all along, as he did later? It seems it was meant to be significant, but it always confuses me.
