This looks a lot like Acne Vulgaris / Cyst Acne; a severe form of Acne. But I have never seen anything like this. Certain over-production of hormones are linked to Acne, but he may have a rare form of hormonal imbalance that is triggering this further, and perhaps Accutane may have even magnified this imbalance more. It is well known that DHT, androgens testosterone, and others are linked to acne. He just may have over+++production of these. BTW, smallpox leaves blisters. Those aren't blisters.
