No... no one's been revived. There are two stories going on: one is inside church's memories- in which no one can be revived or not, since it's all just a simulation and the only two fully intelligent beings (sorta) are church and tex.

The other, outside story is a very long flashback sequence. There's no revival. Because it's in the past. And thus no one's died. Yet.

Donut isn't dead, if you are a sponsor on the red vs blue website, when you watch episode 13 of revelation, (battle of the exes) then you get a special sponsor ending, where church-epsilon says to end recovery for all units, then it shows donut get up in Valhalla, his injuries minor/healed because he doesn't complain, but he does complain about blood on his "nice shoes"
