The levels you can't normally go to are in fact variations of the original levels. So by editing the original level all the variations of it will also get that change.

THe enemy comes up random everytime you play this level so thats whe it just keeps appearing there and killing him but not always.

If it's the one I think you mean, there's a video about that level. Just type in Super Mario Brothers: Exploring a strange level. Only difference is that the person used a swimcode so he didn't fall to his death.
