He repeats it a few times, he just does it really fast. He uses a similar technique at 52 - 54 secs (Down Down Up)

The song starts out quiet, and as it gets more furious towards the middle you can see him turn it all the way up, right before the first big vibrato bend. He also fiddles with it and turns it up slightly a few times before that I believe.

SO ORIGINAL!!! Seriously, as cool as Zakk sounded when I first heard him, he just doesn't do anything different. Cliche pentatonic licks and some hard rock and doom style riffs thrown in there. That's Zakk in a nutshell and he doesn't ever break out of that nutshell unless he is playing an acoustic guitar.
