Again, what I see in this man is that he tries. However, his shoulders are held too low. His chest is also too low. His neck is too long. He needs to raise his shoulders to 1 to 2 inches within the level of his chin. It is not a nervous gesture. When aligned with a lengthened spine and freed neck, it'll all adjust. It takes months to change to a better held set of shoulders and chest but like anything, practice makes good. I'm sorry but that is how I see it, and it is undeniable.
@BackInsight While perfect posture is a worthy goal, as you said it takes months to achieve. Frankly, I do not have the time nor adequate motivation to do the work to achieve perfect posture. Fortunately, perfection is not necessary to alleviate pain and pain can be resolved fairly quickly. While I agree with you in concept, I think my approach may be more practical for the majority of people suffering from shoulder pain than an in-depth posture-correction program that takes several months.

In my practice I have found much the same and frequently recommend that my clients strengthen their external rotators, lower and mid traps and serratus anterior while stretching their pectorals and upper traps.

This is great. My shoulder's been bothering me and I found myself sitting up straight as I watched this video. Another good reason for good posture.
