Is the dark dude at 00:12 a Dark Momotaros? Is the purple Sword Form at 00:14 an Dark Sword Form?

Yes and yes. From what I've heard (from Kamen Rider LiveJournal, who are sourcing their info from elsewhere) the Dark Momotaros is Negataros. He manages to steal the Den-O belt and transforms with it.
