@DeepakPawndU : I know that Rob Harris died during the production of a soft drink commercial with a "double malfunction"; although I am certain it is a bit more complicated than that. I am not certain how that Patrick died... I assume that he died much the same way. The sport has evolved every year since then... mistakes are still made and equipment (extremely rare now) malfunctions.. but to their credit and those who have also died.. have contributed to making this safer.

i realize this may lack tact... but I am highly interested in wingsuiting (much thanks due to patrick)... does anyone know what went wrong so I can avoid this in the future?
@Kelter82 From what I read it was a rigging error that was made when he tried to modify his equipment. He tried to pull his main canopy and his reserve was actually caught in the same line so the weight meant he was unable to fully open either. I want to do wingsuit flying to, there's places you can learn but you need a minimum of 200 skydives in 18 months or 500 skydives over however many years. Good luck.
To spin like that and come back on heading, without dizziness, and with the altitude changing the pressure in your inner ear... wow. The man was clearly exceptional.
The main had never come out of the bag. Patrick cut away, but the trapped steering line kept the bag and the pilot chute trailing behind him. He would not have had much time. He tried to pull the mess clear but that would not have been possible. He had to pull his reserve and the two pilot chutes entangled. The wings create a huge burble. The reserve was still in the freebag.
