This video is misleading! Patient doesn't need to be conscious for you to check for Kernig's! The proper way to do the test is to FULLY flex the HIP ( to stretch the nerve roots) as well as the knee and attempt passively extending the knee. If the meninges are inflamed, the nerve roots will be irritated and cause the hamstrings to go into spasm preventing full knee extension and pain. The key finding is inability to fully extend the knee and not the PAIN!
the elevation of the other leg on hyperflexion of the knee confirms the kerning+
you got also the flexion of the 2 legs (ankles on the doctor's hand) to about 90° any resistance or if the legs extand at the knees before you bring them to 90°angle , it confirms a kerning +
