all character windows must be open and empty and team battle must be on. then press L + R + start to save the settings. go to melee again and perform the name entry glitch using only one controller.

You need to go back to the Vs. Mode screen while on the same frame, you need to try to enter your name. You can do this by holding in B while selecting a name, then after a certain amount of frames you press A to enter a name (as he does at 0:10). The game will glitch and move to the Stage Select Screen.

Masterhand is the same, just don't select a character before doing the glitch.
