It's all mental. He thinks he feels Charlie slapping him, but it's a delusion. Notice how whenever there's an imaginary person, they never interact with their surroundings. For example, when his imaginary friend convinces him to break out of the hospital, Hurley opens all the doors and the window. If it were the imaginary person, Hurley would end up slamming into the door.

unles im mistaken when she says "the hatch blew desmonds clothes off and now he can see the future", im sure he could see the future before the hatch imploded when he did that crazy thing where he went into the past and that creepy woman mentions the guys red shoes then he dies. i thought that was when he first could see the future??(he was in the bar and stuff) does anyone know?? i get way so confused =S
