Long story short, it's an incredibly painful disease that will eventually be the death of her (this explains why he "can't help her now," etc). He's just singing about how he feels helpless against her being sick, and he's breaking too. But it's beautiful. :-)

I've always loved this song and I've just recently been able to listen to it again because a week or so before my ex broke up with me we sat on the couch, forehead to forehead and sang this together. It was after we had sex (lol). But he told me that I didn't have to cry or hurt anymore because he was there for me. And that he didn't want me to have to make my diamonds fall.
