The third ghost orb will probably never be possible in a real game.
There is a trick you may be able to do to consistently get 3 orbs. In the video, the last rune manifested automatically, but if you don't buff with WMB at the start, you can force a rune at any time.
In the video, I did not level asura spirit crescent in order to get a rune without needing to charge, but since Rebirth lowered the max charge time in pvp, it may be possible to get 2 ghost orbs with asura spirit crescent.
