At the part where James sees the blood on the road and he says "That shadow just now...".

Well I read on the official website that if you play with with recommended brightness setting. Which it tells you exactly how to set it in the brightness menu in the options.

So with the correct brightness, that patient would actually really look like a shadow. Try it, it works. Also makes the game 100 times more scarier. :D
