It's either a Letterman look-alike, or a special effect as was used on episodic TV shows such as "Bewitched," "The Patty Duke Show," and others that required one actor to play two or more roles on screen at the same time.
The closer I look at it, the more I'm convinced it's only a Letterman look-alike/impersonator.
Actually, I now see the date of this actual episode of "Late Show" is 5-20-1994, so 15 years ago. Naturally, a person's appearance may change a lot in a decade and a half!
If you want to see Shaffer look even more "different," try watching some "NBC's Saturday Night" (before it was re-titled 'Saturday Night Live") shows from the late 1970s and early '80s, where Shaffer was musical director.
In a few, memorable sketches, Shaffer also played rock concert impresario and legendary record producer Don Kirshner. Perhaps the most famous of those was Shaffer as "Kirshner" introducing The Blues Brothers (Dan Ackroyd & John Belusi ).
But the ones I most fondly recall are Shaffer's "Kirshner" (one with "Not Ready For Prime Time Players" cast member Gilda Radner playing "Kirshner's" wife) introducing Radner as "Candy Slice," her impersonation of punk rock singer/poet Patti Smith..
In one of those "Kirshner" skits, Radner as "Slice" actually combed her underarm hair, as a goof on singer Smith's very visible armpit on the cover of her "Easter" album.
It was an absolutely hysterical bit of comedy, and contributed to making Radner among the funniest and most outlandish of all the "SNL" players.
It was so sad, almost tragic Radner died so young from ovarian cancer. Many people thought she would become the next Lucille Ball, Imogene Coca or Carol Burnett, a true clown princess of TV comedy!
But Radner's wishes, that her premature passing stand for something positive, have been continued by her husband, actor-director Gene Wilder, establishing "Gilda's House" for other ovarian cancer patients.
Tangential trip down "Memory Lane" aside, as noted by your comparison of Shaffer's appearance in this video, the "Late Show" band leader has now allowed his hairline to "fully recede."
By the way, you may now obtain those earliest episodes of "NBC's Saturday Night"/"Saturday Night Live" again on home video (DVD).
But if you just wish to see Paul Shaffer with more hair, and one of the Gilda Radner/"Candy Slice" sketches to which I referred, you may also check out the "Gilda Live!" DVD.
That's not the one with Radner/"Slice" doing her armpit hair-comb ( I still need to research for the specific episode of that one). But it's a good, overall look at Radner, and with Shaffer.
now how the hell he make the fun i don't understand i mean from watching the tv ok thats funny but the viewers over there the live viewers how they see that.. u need magicians to do that
Which one is actually Creepy Dave? It's kind of like how many licks to the center of a tootsie pop, no one knows.

OMG Paul looks so different!

Paul Shaffer hadn't shaved what was left of the hair on his head, and this video is from a few years ago--that is all.
Actually, to most viewers who have watched David Letterman's programs for many years, Shaffer looked as he does in this video most of the time.
Have you never known or seen a person before, who is losing his hair, shave it all off to have a more uniform appearance?

It's funny how Letterman used to leave his glasses off, in an attempt to look less like a nerd, because he'd spend the entire show blinking.
