I have heard of others using a dremel, but it increases the chance of chipping or breaking the ear. Doing in by hand is a bit safer for the doll and a litle less scary!

I use a dremel to do all mine and with an engraving bit, though a pin vice sounds pretty handy and doesnt deal with spending a lot of money, awesome!
