With all due respect to this doctor ,it did look like the old man was missing an eye in the first place. I presume that in similar situations with proper medical care, a regular eye socket flushing of some sort would prevent decay of existing tissue and keep the flies away. It's obvious to me that this man lives a hard life, and probably would explain the missing eye in the first place.
@psychotroniko All it takes is living in the "right" part of the world, where these sorts of flies live... and bad luck for human maggot infection is rather rare, especially in the eye sockets.
