Actually, it is before you are defiantly pregnant. When you try for baby, there should be a chime. If you hear the chime, go eat for whichever gender you want immediately and keep eating until visibly pregnant.

If you get your preggo sim the Fertility Treatment reward, you have a more likely chance to have twins. If both parents get the treatment, it'll increase your chances of having triplets. Whether you want multiple births or just one baby, you can chose the sex by giving your pregnant sim apples (for boys) or watermelons (for girls).
