Isaac has a necromorph head because its one of the ways he can die. An Octopus like thing rips off his head, replaces it with its own and manipulates Isaacz body O.o

one of the deaths is the head necromorphs things when it grabs hold of u, if u dont do any thing, it rips ur head off and makes its head ur own. then isaac turns around and walks off screen
