Yuyuko is only the first boss because she's testing to see if the players are still strong enough since the last game, notice how her clothes aren't damaged like most other touhous after you beat them.

So... the life system sucks if you can't get many of them? Most shmups don't give you more than three or four extends and that's only if you do good.
