In terms of the public, everyone knew as it was revealed earlier that year at Zoe's birthday party in a scene upstairs just between Charlie and Kat. The big shock of this cliffhanger was not the information itself, the audience already knew, it was the shock of it finally being revealed to Zoe.

I find it odd that Kat hasnt mentioned Zoe at all in her current pregnancy storyline, you'd think she'd say something about her previous pregancy!
