@TheMrminionman You are correct. No one actually knows all the conditions to make her appear. I made her appears twice on two different characters the first time I was in the poison swamp, she was waiting there for me. Both character had neither +10 pyromancy flame nor were Pyromancer starting class (one was Wanderer, the other was Knight)
She's not always there. Apparently there's a chance you will find her without a +10 flame, but the only GUARANTEED way to find her is getting the +10 flame.
Just got her to appear today. Heres what triggered it for me. Leveling my Flame from +8 to +13. I Didn't check right away, but about a hour after doing so, i went down and saw her.
Lv 64, Both bells rung, only extra bosses slain are, Butterfly, Hydra, Red Dragon on the bridge.
Have not been to sens fort, nor killed ceaseless discharge.
Also I have not joined the chaos covenant, as i want to wait till I have the witches ring first.
@Umezawa91 You'll find her at the bottom of Blighttown, right in front of Quelaag's domain - she is sitting on a pillar island. Although she will only appear if you have a Pyromancy Flame+10 in your inventory.
@Umezawa91 So when your head finally turns into an egg, go talk to the egg-guy near the spider lady and he'll give you a pyromancer flame. He can upgrade your pyromancer flame up to +15 I think, and Quelana will turn this pyromancer thing +15 to a special pyromancer flame.
@Umezawa91 oh man... welp, only way for you to get a pyromancy flame thing is to either re-roll a character or just beat the game. You'll have a better chance next time during a New Game + !

At the entrance of Queelag's Domain, you'll see 2 eggs enemy, just attack one and get closer to him, he'll do a grab attack. Your character will scratch his head from here and there. Wait 4 minutes and your head will turn into an egg.
@Umezawa91 When you killed the eggman near the spider lady, he should have dropped an egg vermifuge. Check your inventory to see if you have it. If you don't have it, then you'll have to kill many many egg-dudes who are in the quelaag's domain for them to drop an egg vermifuge. Otherwise, the Two Headed Snake Lizards enemy in Darkroot Garden drop the egg vermifuge as well.
@starslayer159 There are many requirements needed to fulfil in order to make her appear - she can appear even without fulfilling them. This is really frustrating since the official guide is wrong on many things: for Quelana's appearance, it says you need to have a pyro flame +10... but many people were able to talk to her even though they didn't have the pyro thing +10. I didn't want to lie, I just merely quoted the official guide.
I've not killed Quelaag nor did I start with pyromancy, yet I still see her. Top comment has something off.
