Clean up your mesh,it looks like you got triangles around the nose. convert them into quads cause triangles come out to ugly effects like such seen here

the motion tracking is great for that time but the mesh isn't clean. it probably has triangles around the nose area.

it wrinkles in unnatural spots and patterns, unlike the actual source video. it's still a step up from the application without mapping, though.
