While riding in Mill Valley, I saw a bike lane that was routed onto a wide sidewalk for a block. I'm beginning to think that this is the solution.

it's a tough call. I'd say the sharrow has to go in the middle of the lane or else on the sidewalk.
if there wasn't a gas station right there, a curb cut to continue the bike lane might work, but would be expensive.
