Shiva was not killed. Kali was soo carried away by her dance that only her husband could stop it before the world was destroyed so he lay in her path and when she put her foot on him she stopped and stuck her tongue out in surprise hence the pictures of Kali with her tongue out.

The reason for the tongue sticking out is diferrent. Each drop of blood that fell to the gorund spawned more demons. So she slurped each of the drop off such that it would not fall . Hence the blood and tounge sticking out.
