He does admit to his heroin use in the 1994 interview disc 'Disturbed'. Its an unofficial release where he talks about touring and making the DS album but you can buy it.
He says, "...and I experimented around with Heroin this year. But my overall view of the drug is that its just too shitty."

This was the last great year of the MTV Video Music Awards. I think this was also the year they had a super pissed off Rage Against the Machine, as well as I think a class act from Kid Rock, Run DMC, and Steven Tyler and Joe Perry from Aerosmith as the introduction.

i got into nin during the year zero peroid some of my fav songs from their but the first time i unoffcciouly heard nin was jhonny cashes cover of hurt but im into almost everysinge nin song out therer i luv nin i luv there music i luv trents energy hes more active then bands hafl his age way more energy anyways i luv nin trent and aaron are my two heroes for me trying to learn guitar,bass,drums, and of course piano imj a nin fan and proud of it

I wanted to check this version out again after seeing his most recent live version of this song he performed in Australia. Its interesting to see the same song in two different periods.
