Of course they have pretty big fanbase in Europe but so do many bands.I suppose that they may be even more popular in the USA than in the UK where at the moment bands like MUSE and Kings of Leon are huge. the thing with Placebo is that they do not make universal, intelligible music so they will never be as popular as bands that set the bar lower trying to gain as many listeners as possible.

Placebo and Manson are on a completely different spectrum. The reason they're not so popular in america is not because of their image, but because of how honest they are, something americans don't like. They actually acknowledge things like drugs and sexuality, something the americans would rather just sweep under the carpet.
