It's everywhere. I'm a professinal bellydancer and I'll tel you that behavior is almost regular. The only way to avoid it is to dance on a stage. If you dance at restorants/teahouses  you have what you have :(

But then, the main function of maiko/geisha is at parties, not on a stage. Although I guess the big tourist parties and such will have the more background noise than smaller parties, so this may be one of the worst settings for noise. Food on the table compounds the issue indeed.
