I think your suggestion of PRACTICE, PRACTICE, PRACTICE is great! My boyfriend recently came out to me as trans, and I am still getting used to the he's and him's. I have definitely made it a point to refer to him as he wishes. And like you and guys, we have a similiar relationship. He's always been more male to me than femme, so it's not so drastic. Today, I slipped up twice... quickly corrected myself and moved on. I notice that the more I say it, the more other ppl say HIM, and HE as well.
I think the reason some people don't change the gender pronouns they use, is because they don't want to. It messes with their whole perception of everything society has taught them about gender.
It's really easy to switch from "she" to "he", when you actually try.. but yeah, some people don't want to.
I have a MtF friend who I satrted using "she" and other people started doing it too... my motivation though was also that I'm the same and wanted people to do it for me
