The report is useless, it does not indicate if it was his main or reserve that got caught or if he deployed the other before landing. People can and have died under fully inflated and functional parachutes, most bad accidents are caused by mistakes in the last few hundred feet.

I'd say his chute opened prematurely, sucking him out right into the tail. It can rip the tails off of planes, that's why everyone immediately jumped and it's also why the pilot wears a "bail out chute" since pilots normally have never seen a parachute. He pulled his cut-away tab and then used his reserve chute to land. It happens. Not often but it happens. I just started skydiving, if it happens to me I hope I can react as well as this guy.

During WW11 Sgt. Nicholas Alkemade, a rear gunner in a Lancaster bomber couldn't get to his parachute as his aircraft was shot down. Rather than burn to death he decided to make it a quick death and jumped. He landed in a thicket of trees and then into a snow drift. He survived without a scratch. Bail out altitude-18,000ft.

But you are right, terminal is reached at around 1200 of freefall from a stationary object. we get there a bit faster on a jump from a plane due to the speed of the aircraft from which we jump.

No i've seen another video with a same situation, only there you could actually see the fall. It makes me think if it would be the same guy and the same event. But in that video the guy survived too from about the same height, also damaged parachute i think but it was really near freefall speed.
