The first 3 are default for superstar training skills.

To unlock the 4th option, which is "Freak" you need to unlock more madden rings. You can see how many rings you have in the middle shrine at the main menu. Some of the more popular superstar positions, like QB and RB, require that you unlock all 4 rings, while others like OL and DB, only require the first one or two rings. Hope this helps :)
